Physician Form-Signing Fees
OHIP does not cover physician fees for completing ODSP medical forms, disability insurance forms, or DTC certificates (T2201). Ontario doctors typically charge $75–$200+ per form. Some community health centres and family health teams waive or reduce these fees. Always ask your clinic in advance. If cost is a barrier, contact your local ODSP office — they may have resources to help.
Frequently Asked Questions
Can I have life insurance while on ODSP?
Yes. Life insurance policies (including whole life with cash value) may be exempt as an asset under ODSP rules up to certain limits. Term life insurance premiums are generally not considered income. Always verify with your ODSP worker.
Does disability insurance affect ODSP payments?
Yes — if you receive disability insurance (e.g., from an employer or private policy), those payments are considered income and will reduce your ODSP entitlement dollar-for-dollar above the exemption threshold.
How much does a doctor charge to fill out ODSP forms?
Physicians in Ontario typically charge $75–$200+ for completing ODSP medical forms (OHIP does not cover this). Some community health centres provide this service for free or reduced cost.
What is the Disability Tax Credit (DTC)?
The DTC is a non-refundable federal tax credit for people with severe and prolonged impairments. It's also a gateway to the RDSP, Child Disability Benefit, and other programs. Apply through CRA Form T2201.
Can I work while on ODSP?
Yes. ODSP has an employment earnings exemption — you keep the first $200/month plus 50% of anything you earn above that. Employment Supports through ODSP can also help with job training and equipment.
